TREATING SEVERELY ATROPHIC MAXILLA

In 1965 Per Ingvar Branemark performed the first ever dental implant surgery on Gösta Larsson, a Swedish edentulous patient that was born with severe chin and jaw deformity. Mr. Larsson died 40 years later with those implants supporting a fixed prosthesis.

Professor Branemark presented his research in 1978 at the world first Dental Implant Consensus Conference where criteria and standards for implant therapy were outlined. He was motivated to find prosthetic solutions for the “ dentate handicapped” that could not be reasonably treated with conventional acrylic dentures.

The posterior maxillary jaw presents challenges in the form of low density and atrophic bone height. These clinical restrictions led to research and development of Pterygoid implants anchored to the Sphenoid bone utilizing longer implants of 18-22 mm in length. Dr. Tulasne introduced his research in 1989 with a cumulative survival rate of 94%.

In 1990 Prof. Branemark introduced the usage of Zygomatic implants as a response to clinical challenges to rehabilitate patients with insufficient bone in the posterior maxilla due to many different reasons such as aging, tumor resection, trauma and severe atrophy of the antrum.

The placement of trans-nasal and trans-sinus implants were incorporated in the treatment of severely atrophic maxillas in the early 2,000s.

SURGICAL PREPARATION PHASE

Dr. Safa Tahmasebi is a Board certified Prosthodontist . He joined the SameDay Dental Implants Branemark Osseointegration Center in Dubai, UAE as a clinical director specializing in full mouth implant rehabilitation and smile reconstruction.

He is faculty of the prestigious Piko’s Intitute in Florida, USA.

Dr. Safa began speaking to audiences in 2013 and continues to lecture at major conferences in the United States and Internationally on full mouth implant rehabilitation protocols.

The surgical preparatory phase of LIT-level 6 will be conducted in Orlando. Florida during April 4-6, 2024.

Dr. Safa will lecture on head and neck anatomy, present step by step of Zygomatics, Pterygoids, Trans-nasal and Trans-sinus implant placement protocols, review clinical cases and management of complications. 

He will also direct a surgical hands-on workshop on cadaver skull PVC models where each attending advanced doctor will place 2 Pterygoids, 2 trans-nasal and 2 Zygomatics. 

PROSTHETIC PREPARATION PHASE

Dr. Mongalo’s textbook “ Prosthetically driven, surgical Implantology” is a reminder that our end goal is to provide our patients with functional fixed prosthesis for patients with severely atrophic maxillas. 

While the immediate and final prosthesis are considered “simple” they are integral part of the equation and it has been proven the one of the major factors in failure of full arch immediate loading cases are the prosthetic techniques followed. 

Dr. Mikal Lindman is a board certified prosthodontist. He has lectured at the ICOI, AAID and internationally teaching his surgical and digital restorative workflow for immediate loading cases of severely resorbed maxillary jaws. 

During the prosthetic preparation phase Dr. Mikal will demonstrate a digital workflow utilizing facial scanners, Photogammetry unit, intraoral scanner, exocad prosthesis design, printing and painting of temporary hybrid. 

During June 6-7-8, 2024, Dr. Mikal will lecture, present clinical cases, show videos and direct prosthetic workshop hands-on skull model where each attending doctor will perform immediate loading on his/her models.

August 11-17, 2024

7 nights of hotel accommodations at the intercontinental hotel

Ground transportation from airport to hotel to Institute

2 days of surgical and prosthetic didactics by Dr. Safa and Dr. Mikal

All attendees will received 3 stereolithography printed models of the 3 patients that they will operate.

1 day of Zygomatic implant placement on stereolithography printed models to simulate the real live surgery.

3 days of live surgeries and immediate loading prosthesis on live patients under supervision of board certificate oral and maxillofacial surgeons and board certified prosthodontist.

Each attending doctor will operate and immediately restored 3 atrophic maxillas. 

84 CE credit hours.

EXECUTION LIVE PATIENT PHASE

Dr. Virgil Mongalo’s vision in creating LIT was based on two solid ideas. First, is to fill the clinical gap that is created by didactic courses where there’s no direct contact with patients. In the past 15 years Dr. Mongalo has proven that this clinical gap can only can only be filled through live surgeries and live prosthetic on patients. 

Second, is the idea that as dentists we must give back to those that are economically underprivileged and that can never afford to replace their lost dentition and bring them a ray of hope through our pro-bono work. 

The live patient execution phase is divided into 3 important components:

A) Review of surgical and prosthetic plan of all patients that will be operated this week in Nicaragua.

B) Live placement of pterygoids, Zygomatics, trans-nasal and trans-sinus implants. 

C) Live restorations of operated cases. 

TRAINING SCHEDULE 

SUNDAY AND MONDAY

Surgical lectures, Prosthetic lectures and CBCT review of all patients that will be operated during the week, emphasis will be placed on proper surgical techniques, case selection, avoiding complications and immediate loading of all cases.

Teams will consists of 1 surgical professor, 1 surgical assistant faculty and 2 attending advanced doctors that will take turns operating on patients.

Each attending doctor will operate one patient per day.

Some surgeries will required Zygomatics, Ptergoids, trans-nasal and trans-sinus blueskybio implant placement. Every surgery will be different and has its own surgical challenges.

TUESDAY 

Dr. Safa, Dr. Mikal and Dr. Mongalo carefully selected a team viabled patients, have printed stereolythography models.

The 6 attendees placed zygomatic and anterior implants on these 18 models. This is the best way to practice the real live surgery on the actual patients models. 

WEDNESDAY – THURSDAY – FRIDAY

7:30am-12:30pm, Live Surgeries and immediate loading will be performed by doctor in group A.

12:30-1:30pm, lunch served at the institute.

1:30pm-6:30pm, Live Surgeries and immediate loading will be performed by doctor in group B.

Learning is a team effort and there’s no better way than to learn from mistakes and successes. This is why each attending doctor will present 3 fully documented cases including surgeries and prosthesis of cases performed this week. 

CASE DOCUMENTATION

All cases will be fully documented by LIT dental professional

photographer, Dr. Angel Acevedo. 

Certificate of 84 CE credit hours will be presented at the end of the course. The best cases will be used and published on Dr.

Mongalo’s “Atlas on Full Arch Therapy” to be published in December 2024.

FACULTY

Dr Safa Tahmasebi

Dr. Safa Tahmasebi received a Bachelor of Science in Biology degree from St. John’s University at Queens New York in 2005. He attained his Doctor of Dental Surgery degree from the State University of New York at Buffalo School of Dental Medicine in 2009. After finishing his general practice residency fellowship at the Montefiore Medical Center in Bronx, NY in 2010, he went on to complete his advanced education program with a master of science in Prosthodontics at the West Virginia University School of Dentistry in 2013.

In 2013, Dr. Tahmasebi joined the SameDay Dental Implants Branemark Osseointegration Center in Dubai, UAE as a clinical director specializing in full mouth implant rehabilitation and smile reconstruction. He has been lecturing on full mouth implant rehabilitation protocols nationally and internationally at major conferences around the world.

In 2019, Dr. Tahmasebi moved back to the United States to join the Metro-Charlotte ClearChoice Dental Implant Center as a key member of the team specializing in immediate loading protocols and full mouth implant rehabilitation.

Dr. Tahmasebi is fluent in English, Arabic and Farsi. In his free time, he enjoys playing soccer and coaching youth soccer, learning Spanish, skiing, and beekeeping.

Dr. Mikal Lindman

Dr. Mikal Lindman received his Doctor of Dental Surgery degree from The Ohio State University. He then pursued specialty training at the University of Louisville School of Dentistry, where he earned a certificate in prosthodontics and Master of Science in Dentistry.

In addition to being a key member of the team at ClearChoice Dental Implant Center in Kansas City, Dr. Lindman has been the Chief Resident of the University of Louisville Advanced Prosthodontics house staff and completed a comprehensive implant residency program focusing on complex implant surgery and advanced bone grafting procedures. He has extensive experience as an owner/dentist in private practice, and has spent time as a clinical director of a dental health clinic in Ohio. Dr. Lindman was a finalist in the American College of Prosthodontists John Sharry Research Competition and continues to volunteer dental care services to orphanages in Mexico.

Dr. Lindman is a member of the American College of Prosthodontists, International Team for Implantology, Academy of Osseointegration, American Academy of Implant Dentistry, American Dental Association, Kentucky Dental Association, and Louisville Dental Society.

Dr. Virgil Mongalo DMD, MS

He is a pioneer in the field of education in implant dentistry.
In 2008 he founded Live Implant Training Institute where licensed dentists are allowed to place and restore implants under strict supervision of accredited professors.
LIT has grown to 3 institutes located in Orlando, Nicaragüa and Mexico. 2,500 dentists have participated on our teaching program and donated 90,000 blueskybio implants to underprivileged population.

Dr. Brandon Kofford, DMD, MS, FACP

Inventor of Smart Denture Conversion Prosthetic Technique.  This process changed the way we perform immediate loading cases. He presented his advanced prosthodontics training to the American Board of Prosthodontists and became board certified. After completing his residency, he earned his Masters of Science Degree at the University of Texas Health Sciences Center and moved to Suffolk, England, with his family to serve as the Director of Prosthodontics and Base Dental Laboratory Flight Commander.
